What is a Facelift?

Facelift surgery, also known as rhytidectomy, is one of the most popular cosmetic surgeries in the United States. Facelift surgery lifts and tightens the skin and deeper tissues to rejuvenate the lower two-thirds of the face.

Facelift surgery tightens loose skin folds, erases wrinkles, and provides an overall younger and more rejuvenated appearance. Ideal Candidates

Ideal Facelift candidates are healthy adult men and women who are experiencing significant signs of aging, including smile lines, double chin, loose jowls, fine lines, and wrinkles. First-time Facelift patients have no minimum or maximum age requirement. Patients, even in their 20s, are seen for a consultation and if found to be a good candidate by Dr. Khan for a Facelift then they would be offered this surgery. A second Facelift may be performed in 10-15 years to maintain healthy, youthful skin.

The Facelift Procedure

Dr. Khan specializes in the superficial muscular aponeurotic system (SMAS Facelift), which provides long-lasting, outstanding results. She performs SMAS Facelift surgery using general or local anesthetic during an outpatient procedure at the accredited Houston Plastic Surgery Center, connected to the Body By Ravi Plastic Surgery and Aesthetics office in Houston, Texas. During the SMAS Facelift procedure, Dr. Khan creates inconspicuous incisions that are hidden inside and along the earlobe fold, behind the ear, and extending into the hairline.

During the procedure, Dr. Khan separates the skin from the thin layer of fibers covering the anterior face and neck, known as the SMAS system. After lifting the skin, she trims the tissue and fibers below the face and attaches the muscles at an elevated level for tighter, lifted facial muscles. Dr. Khan uses the SMAS technique to lift sagging cheeks, remove jowls and heavy skin from along the jawline, tighten the underlying tissue and muscle, correct mid-face sagging, sculpt a double chin, and smooth marionette lines, folds, fine lines, and wrinkles.

Dr. Khan may also remove unwanted pockets of fat from the face for contouring or add harvested fat for definition. She uses fat that is harvested from the abdomen using liposuction, which is then purified and injected into the face using a microdroplet technique to achieve a more youthful appearance. The fat transfer is typically very successful, and 75% to 80% of the injected fat will permanently remain.

The surgeon may also create an additional incision under the chin if the patient wants to improve a double chin, remove excess, saggy skin from the lower face, or enhance the appearance of their neck. Necklift procedures are usually combined with Facelift procedures in order to achieve harmony between the face and neck for a cohesive and comprehensive rejuvenated appearance. The Facelift procedure usually takes two to three hours, depending on the complexity of the surgery. Dr. Khan places sutures within the SMAS layers to hold the transformation in place and adds temporary drains.

Recovery Process

Dr. Khan’s patients typically return home the same day shortly after their Facelift procedure to rest and recover. She advises patients to set up their recovery area before surgery with all the items that they will need. The patient must keep their head elevated and apply ice at appropriate intervals to reduce swelling and encourage healing.

The patient typically returns the following day to have the drains removed. The extent of the surgery determines the length and nature of the recovery. Most patients have a close family member or friend stay with them for a few days to assist them during their early recovery.

Dr. Khan wraps a surgical dressing around the face after the surgery, and a compression garment is typically worn by the patient for 7 to 10 days after the surgery. Stitches are usually removed in one week, and the patient may experience some swelling and unusual skin sensations for up to one year after the surgery. Swelling typically improves after 1-2 weeks.

Dr. Khan prescribes medication to reduce the risk of side effects, which can include pain medication, antibiotics, and anti-nausea medicine. The bruising begins to fade within the first few days and disappears in approximately two to three weeks. Patients may resume their regular activities within a week if approved by Dr. Khan, minus strenuous physical exercise.

Facelift patients are encouraged to contact Dr. Khan or the staff at Body by Ravi at any time during their recovery to ask questions or discuss concerns. Most patients are comfortable returning to public events approximately two weeks after the surgery.


There are risks associated with any surgery, but when facelift surgery is performed by an experienced plastic surgeon, such as Dr. Khan, the side effects and dangers associated with SMAS facelift surgery are significantly reduced. Although SMAS complications and adverse events are rare, facelift risks may include:

Depending on the individual patient, the treatment can take from 45 minutes to two hours. While most people find Morpheus8 easily tolerated, a local anesthetic or topical numbing cream will be applied before beginning the treatment, to ensure complete comfort. Buttocks – Better known as the ‘Brazilian Butt Lift’, a fat transfer to the buttocks provides shape and contour to the bottom. Often combined with liposuction of the surrounding areas, a Brazilian Butt Lift will sculpt the buttocks to create a more youthful, natural-looking figure with a perky, round derriere. To determine if a fat transfer procedure is right for you, a confidential consultation will be arranged. Potential patients should plan on spending an hour or more in the Body by Ravi Plastic Surgery and Aesthetics’ office, as our surgeon and staff gets to know your needs and desires. During this initial visit with Dr. Ravi, the surgeon will perform a physical evaluation and gather all pertinent medical history. During the initial appointment, Dr. Ravi will discuss the fat transfer process in detail and explain what donor sites are most desirable for fat removal. After looking through Before/After photographs of his recent fat grafting patients, it will be decided if the technique is right for you. If so, a customized treatment plan will be created, walking you through the fat transfer process, step-by-step. The ideal candidate for fat grafting will be in overall good health and a non-smoker. If needed, Dr. Ravi can provide information and advice on smoking cessation. Potential patients should be at a stable body weight with enough fat for successful transfer.


All fat transfer patients are seen approximately two weeks prior to surgery for their pre-operative appointment. At this time, all surgical consents are reviewed and signed. “Before” photos are taken, to be compared to the “After” pictures taken during and once the recovery is complete. Dr. Ravi will provide an informational packet with detailed instructions for before and after surgery. If there are any remaining questions, Dr. Ravi and his team will provide you with the needed answers, ensuring patients are ready to move forward with their upcoming fat transfer cosmetic surgery.


Fat Transfer Dr. Ravi will perform his fat transfer procedure at an accredited surgery center. The procedure will take from one to five hours, depending on the amount of liposuction, target location and the extent of correction. Patients will be home the same day, resting and recuperating. To begin, Dr. Ravi will harvest the predetermined amount of fatty tissue from the donor area. This is most commonly the abdomen, thighs, arms, or lower back. A local or general anesthetic will be administered before a thin cannula is placed under the skin. Using a sterile liposuction technique, Dr. Ravi will extract the needed fat through a syringe. The tissue is then washed, purified, and separated using a special centrifuge and filtration process. Once the fat cells are processed, they will be ready for placement. The target area will be carefully prepped. Dr. Ravi will then inject the fat using a uniquely designed needle, meticulously achieving the desired contour and volume. The needle will be passed in and out of the area multiple times, creating a line of fat cells deposited within the natural tissue planes, creating a grid of grafted fat. Once the desired correction has been achieved, Dr. Ravi will place a surgical dressing and compression garment for safe and proper healing.


The recovery time after a fat transfer will vary from person to person, depending on the target location, as well as the extent of liposuction and grafting. In general, you can expect swelling and bruising that may last up to three weeks. The donor area is commonly tender for several days. Limitations to physical activity will depend largely on the extent of the grafting. Patients can expect to spend one to two days resting before resuming daily activities. Most people are comfortable returning to work in two to five days. While your normal routine can be resumed in just a few days, more strenuous activities should be avoided for at least three weeks, or until approval by Dr. Ravi. Fat grafting procedures that require harvesting a large volume of fat will require a surgical compression garment be worn for several weeks as well as a more extensive recovery period.


Fat grafting can be extremely successful when performed on its own. However, the technique can be combined with complimentary procedures to achieve optimal results. Techniques such as a tummy tuck or breast augmentation benefit when combined with fat grafting. Fat transfer to the buttocks (also known as the Brazilian Butt Lift, or per Dr. Ravi, “The Houston Butt Lift” with the use of his special techniques) can be performed at the same time as a tummy tuck with liposuction in the appropriate patient. Fat transfer can be added to a breast augmentation with implants to improve the fullness of the upper pole or to add more cleavage. Microneedling and chemical peels are also excellent in office procedures to pair with a facial fat transfer.
