What to Know About Recovery

Recovery from breast augmentation occurs in stages. During the first week or so, you should plan on taking it easy. Avoiding physically demanding tasks and primarily resting will be best during this time. You will need to avoid soaking your incisions, so bathing and swimming will be prohibited during this stage. You will not be able to soak the incisions until they have healed – which may take six to eight weeks.

Dr. Ravi performs electrocautery of the blood vessels during breast augmentation. While more time consuming, this surgical technique reduces trauma and minimizes post-operative bruising, swelling, and pain. Taking this step helps ensure your recovery will be as short and comfortable as possible, but swelling may continue to produce asymmetrical results for a few weeks. During this time, your breast implants will be settling into their pockets. Once settled, your breasts should take on their new look. This process can take several weeks.

Because recovery can take several weeks and, during that time, swelling and symmetry may be present, having breast augmentation performed during the cooler months, when more clothing is worn, may be best. Breast augmentation is the most common plastic surgery procedure performed today. However, not every augmentation provides ideal results. Even with the most experienced surgeon, unexpected problems may occur. After surgery, some women may find their breasts are too large or small, no longer fitting their physical proportions or lifestyle. The implants may be malpositioned, asymmetrical, or sit low on the chest. Additionally, although complications following a breast augmentation are rare, there is still a small chance for issues such as capsular contracture, rippling, rupture, leakage, and deflation. Conversely, other women may have nothing ‘wrong’ with their implants; however they may find themselves wanting to switch out their saline implants for silicone.

What is Breast Implant Revision?

Breast implant revision is a specialized surgical technique designed specifically to correct the issues that can arise following breast augmentation. Who is an Ideal Candidate for Breast Implant Revision?

Almost anyone who is unhappy with the results from an initial breast augmentation procedure may make a good candidate for Dr. Ravi’s breast implant revision. A potential patient should be in overall good health, a nonsmoker, understand the recommended technique, and hold realistic expectations for the final results. What to Expect During Your Breast Revision Consultation

Patients should set aside at least an hour for their breast implant revision consultation with Dr. Ravi. This appointment is an important part of the revision process, as it provides Dr. Ravi time to learn more about your individual concerns and desires. During the consultation, Dr. Ravi will perform an in-depth physical examination, determine the health of your existing implants, and assess skin elasticity. At this time, Dr. Ravi will also carefully review your medical history, current medications, and discuss pertinent health issues.

After listening to your aesthetic goals, Dr. Ravi will explain the various options and techniques available for implant revision. He will show numerous before/after photographs of his recent breast implant revision patients, helping to create a clear picture of the potential results. Once Dr. Ravi has determined that you are a good candidate for the breast implant revision procedure, he will create a personalized treatment plan. If there are any remaining concerns, Dr. Ravi or his patient coordinator will be available to address your questions. Together, Dr. Ravi will work with you in making an educated, informed decision regarding how to move forward with the augmentation revision procedure.

The Revision Mammoplasty Pre-Op Evaluation

Approximately two weeks before the breast implant revision procedure, you will be seen in the office for a pre-op evaluation. This office visit will provide a time to ask any last minute questions of Dr. Ravi or his patient care coordinator. Specific instructions for before and after the surgery will be provided. Dr. Ravi will go over a thorough list of ‘Dos and Don’ts’ for this period. Prescriptions for pain and nausea medications, as well as an antibiotic, will be provided. Please have these filled prior to the procedure. Finally, ‘before’ photographs will be taken and consent forms will be signed.

For a safe procedure, please remember:

  • Avoid aspirin and NSAIDS two weeks before your breast implant revision, as these can cause unexpected bleeding and bruising. Do not resume these medications until at least two weeks after your procedure.
  • Stop taking any and all herbal supplements at least one week before surgery, as these may result in delayed wound healing. No supplements should be reintroduced until at least a week following your breast augmentation revision.
  • Cease all smoking, nicotine, and e-cigarettes for at least a four weeks before your breast implant revision. Nicotine products will decrease circulation, risking serious complications and delays in wound healing. Women should not resume smoking until at least four weeks into their recovery.

How is Revision Mammoplasty Performed?

Dr. Ravi’s breast revision procedures are performed at an accredited surgical center, using general anesthesia, under the care of a board-certified Anesthesiologist.  The surgery will take from one to four hours to perform, depending on the specific techniques utilized and the extent of correction.

To begin the breast implant revision procedure, Dr. Ravi will make thin incisions along the breast as needed based on the chosen procedure. If possible, his incisions will run along the existing scar lines, although this is not always possible. Once the incisions are made, Dr. Ravi will provide the necessary correction. In certain cases, the skin and internal support tissues may not be able to handle the stress of new, bigger implants. If this is the case, Dr. Ravi may recommend acellular dermis matrix for added internal support.

Breast Implant Revision Techniques Include:

  • Implant Exchange – An implant exchange is an excellent option for those women who are unhappy with the size, symmetry, or placement of their original implants, or have experienced a ruptured implant. Dr. Ravi will assist in determining the most appropriate size implant needed to achieve natural-looking, proportionate results. The new implants can be larger or smaller than the original implants, to either maintain or correct your appearance.
  • Capsular Contracture – Scar tissue is expected to form around the breast implant. However in some cases, this capsule will become too thick, tightening and squeezing the implant, sometimes causing a deformed breast shape. Dr. Ravi will soften and remove this thick capsule of scar tissue, creating a new pocket for replacement implants.
  • Implant Malposition – Implants may slide or migrate within the breast pocket, pushing into the breast fold, creating an unnatural contour, making breasts asymmetrical, or ‘bottoming out’ with too much implant sinking to the lower pole. To address malposition, Dr. Ravi will create a new breast pocket, replace the implants, and when needed, add a surgical matrix for additional support.
  • Double Bubble – Implants that sink along the chest wall, protruding past the breast crease, may form what is commonly referred to as a ‘double bubble’. This complication is seen more often in patients with large or heavy implants, tuberous breasts, poorly placed implants, or scarring from previous breast surgery. To correct a ‘double bubble’, Dr. Ravi will replace the existing implants, recreating an appropriately shaped breast pocket. In some cases, adding a tissue matrix can improve strength within the breast, keeping the implant safely in position.
  • Wrinkling and Rippling– Wrinkles and ripples are more common with underfilled saline implants, although they may occur after any breast augmentation procedure. Dr. Ravi can improve the appearance of a wrinkled or rippled breast by replacing the existing implants, potentially placing the new implants behind the muscle, and when the situations needs it, add a tissue matrix for strength.
  • Symmastia – Symmastia is a unique malposition where the breast implants are pushed together across the midline of the sternum, creating a ‘uni-boob’. Dr. Ravi can improve symmastia by removing the existing implants, redefining the breast pockets, then adding new implants.

The exact technique used during your breast implant revision will be determined during your consultation with Dr. Ravi. As this can be a stressful time for many women, Dr. Ravi and his team will walk you through the process, step by step. This ensures a satisfying procedure, and healthy, natural-looking results that can last for years to come.

Recovery After Breast Implant Revision Surgery

Women should expect their breast implant revision recovery to take about two weeks. While most women are feeling good enough to return to work in less than a week, everyone heals at different rates. It is normal to be tired and  sore for the first several days after surgery. It is also common to have fleeting tenderness, numbness, or feel pressure along the chest. Dr. Ravi suggests that women enlist the assistance of a spouse, friend, or family member during the first day or two after surgery, providing help with household chores and childcare.

A compression garment or surgical bra will need to be worn for several weeks, reducing the chance for swelling and providing the needed support. However, residual swelling can continue for up to six months. Strenuous exercise and heavy lifting should be postponed for two to six weeks depending on the exact procedure performed. Through a series of follow up appointments, Dr. Ravi will keep a close eye on your recovery. As you heal, Dr. Ravi will give you the go-ahead to safely resume activities.

BREAST REDUCTION TECHNIQUES

Dr. Ravi will recommend one of several different breast reduction techniques, each of which can be customized to achieve the desired aesthetic results. The right technique for your unique needs will be determined during the consultation appointment.

  • Circumareolar Breast Reduction – incision is made only around the areola–also known as the Doughnut incision.
  • Circumvertical Breast Reduction – incision is made around the areola with a vertical incision down to the breast fold. This is also known as the Lollipop reduction or short scar breast reduction.
  • Wise-Pattern Breast Reduction – the most commonly used technique, it includes an incision around the areola, vertical line to the mammary crease, and a horizontal line at the crease.
  • Liposuction-Assisted Breast Reduction – in a few cases, liposuction alone or in conjunction with one of the above techniques can be utilized.

THE BREAST REDUCTION PRE-OP APPOINTMENT:

Every patient will meet with Dr. Ravi for a pre-op appointment, approximately two weeks prior to their reduction mammoplasty procedure. At this time, Dr. Ravi will provide specific instructions, including the dos and don’ts for the recovery period. Prescriptions for an antibiotic, anti-nausea medications, and pain medications will be provided during this visit; these should be filled prior to the date of surgery. Pictures will be taken and this is your opportunity to ask any last-minute questions before consent forms are signed.

For a safe surgical procedure, and successful recovery, please remember:
  • Stop taking aspirin and NSAIDS, or nonsteroidal anti-inflammatory drugs, such as Ibuprofen, one week before surgery. To prevent unexpected bleeding and bruising, remain off these medications for at least a week after the procedure, as well.
  • Herbal supplements should be stopped a week prior to the breast reduction. To prevent delayed wound healing, women should not plan on resuming herbal supplements until one week after surgery.
  • Smoking and all nicotine products, including e-cigarettes, must be stopped four weeks before surgery. Do not resume smoking until at least four weeks after your breast reduction. This is vital to minimize the chance for complications.

THE BREAST REDUCTION SURGERY

Dr. Ravi’s breast reduction will take from two to four hours to perform. General anesthesia is used for the outpatient procedure, using a board-certified anesthesiologist. However, in some cases, an overnight stay may be recommended, depending on the individual patient and the extent of correction.

Depending on the exact chosen technique, Dr. Ravi may begin by making an incision, around the nipple then vertically down the breast, and across the breast crease. From here, excess tissue and skin will be removed, while the breasts are lifted to attain a more appropriate size and shape. An internal support system is created from the excess skin, providing a “permanent bra” to increase support and perkiness for years to come. Once the excess tissue is eliminated, the breast and nipple are lifted and reshaped for the desired effect. The nipple remains intact with the essential underlying blood and nerve supply. However, be advised that there is a small chance of inability to breastfeed as well as nipple numbness after the procedure. Dr. Ravi additionally uses internal pedicle-shaping sutures which allows for an improved aesthetic shape to the breasts.

Once the desired results have been achieved, Dr. Ravi will suture the incisions and place a compression garment to encourage healing while reducing swelling.

GETTING BACK TO NORMAL AFTER A BREAST REDUCTION

Initially, women can expect to be tender and sore after their reduction mammoplasty. Dr. Ravi suggests having a spouse, friend, or family member’s assistance for at least the first 24 hours following surgery. Mild to moderate discomfort and heaviness may be felt across the chest for several days. While you may be sore, patients are encouraged to begin walking the afternoon of surgery. Within 48 hours, most women are feeling well enough to perform light, everyday activities. Many are back to work after one to two weeks; however everyone heals at a unique pace. Strenuous activities and exercise should be postponed for four to six weeks.

Patients are required to wear a support garment to help support the breasts after surgery. Drains may potentially be placed during surgery, depending on the size of the reduction. If so, these are generally removed during the first in-office follow-up appointment. Swelling is normal and can be expected for several weeks.

It is very important to sleep only on your back for at least six weeks after the breast reduction surgery. Due to normal breast swelling, patients should wait three months after surgery to be fitted for a new bra. During the healing process, Dr. Ravi and his team will be available to answer any questions that arise.

What is a reduction mammoplasty?

A breast reduction, sometimes referred to as a reduction mammoplasty, is a surgical technique designed to decrease the size of large, heavy breasts. Dr. Ravi’s breast reduction is uniquely created to improve the sagging associated with large breasts, ensuring the chest is proportionate with the body type for a natural-looking result.

Who is a good candidate for breast reduction at Body by Ravi Plastic Surgery and Aesthetics?

Almost any woman who is in overall good health, yet struggling with heavy, pendulous breasts, may make a good candidate for a reduction mammoplasty. The ideal patient will understand the breast reduction procedure and have realistic expectations for her final results. Many women have neck, back and shoulder pain as well as rashes and shoulder grooving which is relieved with a breast reduction. Some women may not have these symptoms but their large breasts cause embarrassment and unwanted stares. They may even have difficulty exercising and participating in certain recreational activities.

Where are the incisions placed during my reduction mammoplasty?

To begin the breast reduction, Dr. Ravi will make an inverted T incision along the breast. This thin incision will run around the nipple/areola, straight down the breast. In many cases, Dr. Ravi will include a horizontal incision across the breast crease, as well. This allows Dr. Ravi to successfully excise excess glandular and fatty tissue, as well as to remove unwanted skin, creating the desired breast shape and height.

Will my breast reduction leave scars?

Yes, patients will have thin scars along the incision lines, after a reduction mammoplasty. However, the extent of scarring will depend on the individual patient. In most cases, the scarring will fade slightly over time, although it does remain visible. Breast Implant Options:

Every woman is built differently, meaning the implant that looks proportionate on one woman will not necessarily be ideal for another. Dr. Ravi will take into consideration each woman’s height, weight, exact chest measurements, skin characteristics and the amount of existing breast tissue before recommending a particular implant. He will also take into account her age and lifestyle. Dr. Ravi only uses FDA-approved implants from Allergan, the maker of Natrelle gel implants. These modern implants come in a variety of styles, meaning every patient will have personalized results. Common implant options during the breast augmentation process include: 

Silicone vs. Saline Implants:

Silicone implants are the most popular choice today. A safe and natural-looking option, silicone implants have a lower deflation rate than saline implants. Prefilled with a soft silicone gel, the results achieved with silicone implants look and feel more like natural breasts. Silicone implants have a lower rupture rate versus saline implants.  If there is a tear in the outer shell, due to the nature of these cohesive gel silicone implants, the material will stay within the implant.

Saline implants provide firmer, rounder breasts that may look less natural than silicone. Made with an outer silicone shell, saline implants are filled with a salt-water solution after being positioned on the chest. This means a smaller incision. While leakage is not common, it is easier to detect in saline implants. However, rippling can be more common in saline implants.

Over the Muscle or Under the Muscle Placement:

Subglandular placement of implants, which is over the chest muscle, will provide a rounder, more-natural look to the chest. This placement typically requires a larger amount of natural breast tissue to be present prior to the augmentation procedure. Subglandular placement can mean a slightly higher chance for rippling, but can be used to improve the position of slightly droopy breasts.

Submuscular placement, under the pectoralis major muscle, provides excellent upper pole fullness, with a contoured shape above the nipple. While submuscular placement is great for women with little natural breast tissue, there is an increased chance for animation distortion, or changes in breast shape with certain movements. These implants may be harder to feel when under the muscle, but can provide better images during mammography.

Volume, Shape, and Texture:

The volume of an implant is measured using CCs; never cup size. A large variety of CCs are available in each type of implant. The use of implant sizers can be a helpful tool for women to visualize how each CC implant will look on her body, allowing her to choose the right volume for her physical proportions.

Breast implants come in both round and anatomic, or teardrop shapes. Round implants are the most popular choice today and provide a desirable, organic shape to the breast. The teardrop shaped implants are excellent for contouring the chest when little breast tissue is present, especially during breast reconstruction. However, anatomical implants can rotate in the breast pocket, taking away from the natural appearance.

Finally, women will be given the option of smooth or textured implants. Smooth are most popular, as they move easily within the breast pocket. Textured implants were once thought to eliminate the chance of capsular contracture, although this has not been found to be the case. However, textured implants may be the recommended choice if this is a secondary breast augmentation.

Your Breast Augmentation Pre-Op Appointment:

Every woman who chooses to have a breast augmentation with Dr. Ravi will have a pre-op appointment scheduled approximately two weeks before surgery. During this visit, pre-operative pictures will be taken. A detailed information packet will be provided and Dr. Ravi will discuss the specific instructions for before and after the upcoming breast augmentation surgery, including a complete list of ‘dos and don’ts for recovery. Prescriptions for an antibiotic, pain, and anti-nausea medications will be written, giving the patient ample time to fill before surgery. Any last minute questions and concerns will be addressed, and all surgical consent forms will be signed.

To prepare for the breast augmentation, Dr. Ravi suggests patients stop taking aspirin and other non-steroidal anti-inflammatory drugs (NSAIDS), such as Ibuprofen, at least one week before and one week after surgery. This will help to prevent unexpected bleeding and excessive bruising. Women should stop taking any herbs or supplements at this time, as well, as these may cause a delay in the healing process. To prevent complications, smoking must cease for at least four weeks before and four weeks after surgery.

The Breast Augmentation Procedure:

Dr. Ravi performs all his breast augmentation procedures as outpatient surgery, at an accredited surgical center or hospital, using a board-certified anesthesiologist and the most innovative, state-of-the-art technology available. Patients are at home resting the same day. Performed under general anesthesia, the patient will need a ride home from the surgical center. Depending on the individual and the technique performed, the breast augmentation procedure will normally take from one to two hours.

To begin, Dr. Ravi will create a predetermined incision along the breast. Most commonly, the incision will be made across the breast crease (inframammary), around the areola (periareolar), or under the arm (axillary). Through this incision, he will then place the implants. Silicone implants are prefilled, however saline implants are positioned prior to being filled with the saline solution. After the implants are in their desired location, Dr. Ravi sutures the incision and put a compression garment in place for proper healing.

Due to his gentle, precise approach to breast augmentation, Dr. Ravi’s patients are known for having a smoother and shorter recovery period, compared with traditional procedures. Keller Funnel® Implant Insertion:

The Keller Funnel® is an innovative tool that can be utilized during the insertion of silicone implants. While not all doctors choose to use this technique, Dr. Ravi finds that using the Keller Funnel® has multiple valuable benefits, including:
  • A smaller incision can be made even when placing a large, silicone implant. This means a smaller scar compared to an identical breast augmentation without the use of the Keller Funnel®.
  • Known for its “No-Touch” technique, this minimizes infection rates and lowers the incidence of capsular contracture (internal scar tissue). The Keller Funnel® prevents the implant from coming into contact with the skin, allowing the integrity of the implant to remain protected.
  • Less pain during recovery, as the silicone implant is placed with less stress or trauma to the surrounding tissue.

Minimal Breast Scarring:

Dr. Ravi takes meticulous care in planning the final results of each breast augmentation procedure. Prior to surgery, he will measure and locate the exact position of the surgical scar, based on the individual’s unique anatomy, ensuring the resulting scar will be well-concealed within the natural breast fold.

Taking extra time in the operating room, Dr. Ravi performs electrocautery of the blood vessels, as he makes the breast pockets for the implants. Many other surgeons choose the faster, more simple method of blunt bloody dissection. While easier for the doctor, the dissection technique results in increased bleeding, bruising, and post-op pain. Dr. Ravi’s slow and meticulous methodology means a faster healing process with fewer complications.

Exparel® for Rapid Healing and Comfort:

Dr. Ravi will prescribe the necessary post-procedure oral pain medications for a comfortable breast augmentation recovery. However, to ensure maximum comfort, he will directly inject Exparel® (a local anesthetic) into the breast tissue during the augmentation procedure. Applying this local anesthetic dramatically decreases the need for pain medications after surgery, so that Dr. Ravi’s patients can recover with less stress and improving the overall healing time. Most women are able to return to work in just two to three days after their breast augmentation, although every patient’s recovery is different.

Getting Back to Normal After Your Breast Augmentation:

Women can expect their breast augmentation recovery to take about a week. Patients will be able to walk immediately after surgery and perform many routine activities by the following day. Subglandular placement, over the muscle, is generally a faster recovery and may take only a couple of days. Submuscular placement, under the muscle, may require a few extra days of rest. Either way, most patients are feeling good enough to return to work or school in a week or less. Heavy lifting, strenuous activities, and vigorous exercise should be postponed for three to six weeks.

For the first 24 hours, Dr. Ravi suggests having assistance with a responsible adult, who can provide any needed help. Women may experience mild to moderate discomfort and tightness across the chest over the first week. Dr. Ravi will prescribe the necessary medications for a comfortable recovery. The Exparel® injections, placed along the incisions during surgery, will help to decrease the pain for up to three days. Bruising and tenderness are common, but generally diminish over the first seven to ten days. The majority of the swelling will resolve over the first several weeks, however residual swelling can remain for six months or longer. A compression garment will need to be worn for the first few weeks, reducing the incidence of swelling, and ensuring the implants move into their desired location on the chest.

At first, new breast implants may sit high on the chest, but as the weeks go by, they will settle on the chest. As the residual swelling resolves and the implants move into the breast pocket, they will take on their desired natural appearance. Modern breast implants are designed to last for years to come, and can easily remain safe and shapely for a decade or longer. Together with Dr. Ravi, patients can safely choose to have a revision augmentation should they want to update their appearance with new implants in the future.

Breast augmentation is considered a safe and effective procedure, especially when performed by an experienced, board certified plastic surgeon like Dr. Ravi.
