At your initial consultation, Dr. Ravi Somayazula performs a thorough medical evaluation to determine good candidacy for breast reduction surgery (also known as reduction mammoplasty). A physical examination is performed to evaluate your current breast size, shape, tissue characteristics, and symmetry. Dr. Ravi Somayazula outlines a realistic treatment plan by using a precise sizing system, which takes into account your exact breast and body measurements, along with your overall body proportions. Breast Reduction Techniques
- Circumareolar Breast Reduction – incision is made only around the areola–also known as the Doughnut incision.
- Circumvertical Breast Reduction – incision is made around the areola with a vertical incision down to the breast fold. This is also known as the Lollipop reduction or short scar breast reduction.
- Wise-Pattern Breast Reduction – the most commonly used technique, it includes an incision around the areola, vertical line to the mammary crease, and a horizontal line at the crease.
- Liposuction-Assisted Breast Reduction – in a few cases, liposuction alone or in conjunction with one of the above techniques can be utilized.

Who Is a Good Candidate?
Candidates for breast reduction surgery should be in overall good physical and emotional health with no history of irregular mammograms, severe obesity, diabetes, smoking history, blood clotting disorders, heart disease, or wound healing disorders with a tendency to develop excessive scarring. Candidates should have realistic expectations of surgery results, and, in some cases, a medical clearance may be needed from your primary care physician.
All patients are seen approximately two weeks prior to surgery for their pre-operative appointment. During that visit, all surgical consents are signed, photos are taken, and a detailed informational packet is provided with specific instructions for before and after surgery. A complete list of “dos and don’ts” is reviewed. Patients receive prescriptions for an antibiotic, as well as pain and anti-nausea medications. Be prepared to follow all recommended pre-surgery and post-surgery instructions closely to ensure your best possible results.
We recommend stopping aspirin and all other non-steroidal anti-inflammatory drugs (such as Ibuprofen) for at least one week before and after surgery to prevent excessive bruising. Herbs and supplements should also be stopped for at least one week prior to surgery as this can interrupt healing. All patients must stop smoking at least four weeks before surgery and four weeks after surgery to minimize healing complications.
Dr. Ravi Somayazula takes about two to four hours to complete a breast reduction procedure under general anesthesia at an accredited hospital with a board certified anesthesiologist overseeing your care. Patients typically go home that day, but some patients do stay overnight depending on the size of the reduction. Excess breast tissue is removed while the breasts are lifted to attain a more appropriate size and shape. An internal support system is created from the excess skin, providing a “permanent bra” to increase support and perkiness for years to come. Most often, an incision is made around the nipple or areola that extends down the front of the breast vertically to the breast fold. At times, an additional horizontal incision is made at the breast fold in order to remove a greater amount of excess skin and breast tissue. Once the excess tissue is eliminated, the breast and nipple are lifted and shaped for the desired effect. The nipple remains intact with the essential underlying blood and nerve supply. However, be advised that there is a small chance of inability to breastfeed as well as nipple numbness after the procedure.
A responsible adult should stay with you for 24 hours immediately following surgery. Most patients do feel discomfort at their chest which can last several days after surgery. Patients are encouraged to get up and walk the same day of surgery. You can expect to perform everyday basic activities, including showering, two days after surgery. The recovery period is approximately one week, but every patient recovers differently. Most women return to work in two weeks but with strict instructions not to lift anything heavy or participate in strenuous activity for six weeks. Exercise can resume after about six weeks. Patients are required to wear a support garment to help support the breasts after surgery. Drains may potentially be placed during surgery, depending on the size of the reduction. Skin will feel tight as the body adjusts to the new size. Swelling can be expected for a few weeks. Scarring will fade over the next several months. It is very important to sleep only on your back for at least six weeks after surgery. Due to normal breast swelling, patients should wait three months after surgery to be fitted for a new bra.
